Directions for creating a Digital Signature and Submitting:
- 1. Click on the Signature Box
- 2. Click on “Create a new id ” or “Create a new self- sign certificate”-click next
- 3. Click on “Windows Certificate Store”-click next
- 4. Type in Name and Email address –click finish
- 5. Save document (so you will have a copy of what you have filled out and signed)
- 6. Attach document to an e-mail or click “submit” on document
